To configure Jira fields, navigate to Project Settings > Request Types > Checkout
Checkout allows you to customize your order using Jira fields. Fields are added and customize through the hidden Checkout RequestType.

Once your forms have been fields have been configured, they will show up in your Checkout cart form.
